Ignore it. It basically appears when someone is trying to connect but can't or lagged out and this will happen for a few seconds.

Or, the person could be trying to crash your server. But then it should appear in masses.

Players have certain states ( connected, ingame, disconnected etc. )

This unexpected message for example happens when a client thinks he's in the ingame state, sending fire and other messages. For the server, the client is in the disconnected states and because the states are not synchronized, the server will not expect "ingame" messages from a disconnected client.

This can happen because a player timed out, or because the server was restarted, or like yates said, someone is sending invalid packets.
