The problem is that the hotspot isnt working as a router, its morely like a repeater it takes your mobile connection and sends it via wlan to any other device. Same to all kinds of tethering the only difference is that it sends via bluetooth or usb connection

At your first question, hotspot alone is just like a LAN without internet connection, but you can also add a internet connection on it. IF an android can be used to host a server then it wouldn't definitely need a hotspot, just a internet connection and also you will also need a applications to run it.
You cannot port-forward the 3G/4G connection since IP addresses are of each tower cell and not dedicated. Therefore you need to log in there to control the NAT or even be DMZ. Now there is no problem if you do an Android hotspot and play on LAN with other people connected to it.
P.S.: UPnP does not work at any carrier obviously.
@ iLoveUS: i think he dont want to host from his phone. He just uses it as internetconnector for any other device, pc maybe. So he wants to open ports for his dedicated server. Just as the usual way works. So he needs to open ports from his hotspotdevice. But as far as i can see it doesnt work for lte or lower connections. You cant acces the "routercp" on it.
And like i said in the poss earlier its because your phone is just transfering the incoming and outsending packages to another device as hotspot, same as a signal repeater does.

So when you turn on the hotspot on the device, the other device can connect to it via WiFi. If the device who hosts, share a Internet, connected devices can use his Internet.
